From what I see in the picture is that there is swelling on the testicular region. It could be a number of things that range from a testicular hydrocele (I have one, not big enough to require surgery.) to testicular cancer.
Either way you need to see a doctor, what they'll do is examine the area with their hands to check for hard lumps, then they will refer you to a clinic to get a ultrasound of the testicles.
Then the doctor will make the decision on what to do. If you have a large hydrocele chances are they will go in and remove it.
If its cancer then you're fucked.
