Bad Business Decisions?

hey /biz/ I'm trying to find a bad business decision that was made in the past 5 years for a research paper.
It can be bad in terms of being ethical or financially poor.
I tried google but every result is TOP 10 WORST BUSINESS DECISIONS EVER.. and I can not find one in the past 5 years.
Your suggestions would be appreciated.
