I've had really good luck with Osh Park (oshpark.com). $5 per square inch for double sided boards, double that for 4 sided. They take Eagle .brd files (or you can make gerbers). Your boards will be purple.

Dirt Cheap Dirty Boards (dirtypcbs.com) is supposed to be pretty good, but I've not used them yet. Their page says "10 each 5x5cm PCBs $14"

If you're ordering from China, don't pick the cheapest shipping, or you may spend over a month waiting for your boards.

Dirty PCBs is rather good judging from what my hackerspace friends have said. You might not get a fixed amount of boards if you choose the cheapest offer (it's between 5 and 10 boards) but they're cheap as chips.

Also Elecrow is decent.

smart prototyping has 10 piece 2 layer 10x10cm boards for the cheapest price I've seen ($12 USD + $9 shipping). Board quality was pretty good and tolerances are pretty much the same between all the cheap chinese board houses

Osh park is actually quality. The 10x10cm places all are basically frontends to the Shenzen ecosystem.
