Logo text: I don't know for sure, but my guess is that this is custom-made.

Printed numbers: not sure, but my guess is that its a custom bitmap font designed by the manufacturer of the heat transfer printer. Assuming that this is a standalone unit it would in all likelihood be running on some kind of embedded system.
If you could find out the make and model of the equipment that printed this you might be able to locate an SDK on their website that has a TTF of the font buried in it for documentation purposes.
