I'm guessing it was the former, since part of the reason the Emperor wanted Birdy to be the mother of his heir was because of her unique powers (even for Ixorians she's supposed to be uniquely strong, and can even channel her powers into a pure energy form.) She's probably even stronger than Gomez, but lost her previous fights to him because she lacks his speed and specialized military training.
It's curious that Revi wouldn't just take a few skin cells from Birdy or blood samples from a previous fight and create an egg to be fertilized with the emperor's genetic material in an artificial womb, but I guess the emperor wanted a true heir, even if his real body is too old to do the deed and his android body doesn't have the equipment.
I think it was implied somewhere that Birdy and Tsutomu were created from genetic samples of members of the Alterian Royal family, which s why they both share characteristics with the emperor (plus there was that hotsprings arc in volumes 16-17 where Tsutomu's class visits the resort which turns out to be run by refugee descendants of Alterian royalty and one of the girls looks like Birdy with brown hair).
It makes sense for super soldiers like Birdy and Gomez to be sterile, but I wonder why Birdy is functionally asexual while Gomez is able to have a wife and a family like a normal human.
