I'm searching for a birthday card template and this is the best I could find (pic related):

In fact, it's about all I've found. Years ago /po/ had a lovely selection of pop-up birthday cards and I was presented with one featuring a 3D gift box and a cat holding a note card. I wish I still had it but I don't.
