4 base folders: a general reaction pics folder, an anime folder, a boards folder, and a "junk" or "misc." folder.
The general reaction pics folder contains all the reaction pics, mostly unorganized. Some recurring pics with lots of variations (e.g. starecat, sweaty guy, grinning black computer man, etc.) get their own folders. So do certain general themes (a rage folder, a puke folder, etc.). Certain memes like carlos, costanza, ABSOLUTELY, bait, feels (and by extension that dumb frog), and so on also find their home here.
The anime folder contains a whole bunch of animu/mango/2hu pics, mostly organized by show/franchise. It also includes stuff like a cute girls subfolder, a dance subfolder, a nazi girls subfolder and folders for some /a/ memes (the chaika folder is 767 files strong, in case you're wondering). Almost every show has separate "artwork" and "screencaps" subfolders inside its folder, and often there are separate folders for different girls and even more folders for different shippings.
The boards folder contains a whole bunch of subfolders for lots of different boards. The contents of these subfolders changes depending on the board. So, for example, the [s4s] folder is full of [s4s] memes, the /w/ and /wg/ folders contain my wallpapers, the /f/ folder contains all the flashes I have saved, the /v/ folder has a whole bunch of /v/-related pics and separate folders for different games, the /int/ folder contains folders for different countries, and so on and so on.
The junk folder is just that. A ton of pics I saved temporary from google image search or pic I couldn't really fit into other folders (which is pretty rare). It contains a whole bunch of subfolders which have no better place to go. Stuff like my meta 4chan folder (with a moot subfolder), cat pics folder, history pics folder, some comics, general pretty-looking art folder, slavs folder, shrek folder, thread derailers folder, will smith folder... you get the idea.
