Why does lumbar epidural block cause prolonged second stage and bladder distension.

I would think it was due to a loss of sensation caused by the anaesthesia which impedes on the emptying of bladder. Also, because the woman has lost sensation of her abdominal walls, she can't push as harder.

But it makes me wonder why not a prolonged first stage as well.
