Should a gorgon/medusa or some other humanoid monster with a perilous gaze (e.g. ultroloth from Planescape) wield a sword, because fantasy genre conventions dictate that sword-wielders will inevitably lock blades with one another and make eye contact up close?

A trained swordsman will be watching their opponents body movements and weapon.

All they have to do is call attention to themselves. It's instinctual to look at someone's eyes when your attention is drawn to them.

Actually, in my setting, there's a blind 'gorgon' swordsman who was cursed for his pride and known for admiring himself in the reflection of his own blade.
He wears a blindfold even though he could weaponize his gaze because if he sees his own reflection (such as in his own blade or his enemy's) he is petrified as well.

Seconding ranged weapons. It gives them more room to take advantage of someone intentionally not looking at them. The closer you get, the easier it is for them to detect and strike using nonvisual senses like hearing and touch (heat and air movements).

I was going to point out that Katana wielders would be the natural enemy to Gorgons who try this trick. Either they are some kind of blind master swordsman or they simply run towards the Gorgon and then make one cut as they pass their opponent.

Both stop after few more meters and then one or both of them die. It might end with the katana wielder slowly turning to stone (while making some kind of cool dying speech) as the Gorgon suddenly turns into a fountain of blood, or falls down in two large pieces, but either way the Gorgon is going to die.

The Japanese love blind swordsmen because of Zatoichi. He was the main character in a set of really popular movies several decades ago. His thing was that he'd wander around the countryside as a seemingly harmless blind man but was actually a huge badass.
He didn't actually use a katana but that point is moot, he still engraved the idea deep into Japanese popular culture.

Most fantasy settings that adopt the blind samurai aren't much sword-prohibition or the swordsman is already give-no-fucks outlaw so there's no point in having a cane sword.

Depends on the exact stone in question, how much any thin bits (eg fingers) matter if they get smashed, and how long the danger will stay around.

A hard enough stone could have vital organs remain undamaged for a few minutes. Long enough for nearby allies to arrive and deal with the danger.

"Should" doesn't come into it nearly as much as you'd think; most monsters with such powers "should" do a lot of things, but tend not to. Why? Because usually beings with perilous gazes don't have to worry about things like melee combat; they get by just fine. For them to focus enough effort on a melee weapon (or any conventional weapon, for that matter) would require a social, cultural, or other such reason.
