DM Brainstorming Help

In a couple weeks, I'm running a Numenera campaign based on the book Dangerous Visions, which is ripe with material.
One of the stories in DV is called Flies, which is about a man named Cassiday who after being nigh obliterated in a spaceship crash, gets rebuilt by aliens that are studying humans. The one change they made to him is "We made you more sensitive to the feelings of your fellow man."
What they really mean is that he now gorges on the sensations of others without the tax of empathy, so he visits his three ex-wives and proceeds to ruin their lives so he can get tidal waves of raw emotion.
One of them is a recovering addict that he forcibly injects drugs into.
One of them has a precious pet that he strangles the life out of.
One of them is pregnant so he Falcon Punches her.
Aliens show back up, decide this isn't what they want, and restore his empathy so he's now cursed to live with the guilt of what he did to his wives.

My question to the Valian/tg/uardsmen is: How would you adapt Cassiday's condition to an RP? How would you make a player feel like they can get away with anything just to yank the chain on them?
