Even if Mac wasn't infected, there's dark humor in the ending. He either knew he was infected, in which case he knew that he won, or he knew that he wasn't infected, which means that he'll freeze to death for no reason. Similarly, he had no way of knowing what was going on with the other guy. He was in an ultimate no win situation.
