I think its appeal rests on how much the yandere loves their object of affection and the things they would do "for" them in order to maintain the relationship or the status quo.
Something like one-sided "true love" that otaku might be able to relate to.

It might have something to do with insecurity/loneliness as well. People who feel like there is no reason for someone to love them would probably be attracted to the idea of someone who loves them despite that it makes no sense.
