Biggest tweest is that you're a brain in an androids body, and the twist after that is that you're not, and the railroad guys only convinced you and showed you "proof" of it (you'll be shown x-ray stuff and the like, they will by "accident" reveal it after you're trapped in a shaft and suffocates after doing a quest for them, sort of like how the dude in Fallout 3 took you out with the stun grenade ) because they want you to be sympathetic by machines.
If you join The Institute afterwards they'll call them out on being retarded since you being a human all along didn't prove that synths have emotions or can think.
The Brotherhood that's absolutely not the one from the capital wasteland (they always want to remind you how the one sin the capital wasteland are traitors and the such) want to hoard all the synths, break em down since they're just machines, and keep all the tech for themselves (I don't really understood this part, but there's sort of a brotherhood city-state somewhere to the west or something?).
