There would health not based on numbers (like you're with your health at 1 and someone prickles you with a needle) instead it would be depending on your injuries and overall physical health.
You would have to take care of your overall physical health like injuries, fatigue, psyché, hunger, etc.
If you have a low mental health, you would start having hallucinations which could be bothersome since they could make you injury yourself. If you don't eat or drink, you will be slower, weaker and could die.
Depending on your actions this would change the interactions. If you start abusing animals, your character would gradually have less "sympathetic" options when it comes to interaction, if you're a good kid you wouldn't have mean options when it comes to interaction, you would have to gradually do it. A child that helps everyone doesn't suddenly kills a dog, so...
But the orphanage is not the entire game, there is a small town where the orphanage is located and you can go there, if you manage to escape, which would be really hard, keys wouldn't be lying around and you wouldn't find a "fence hole" to get out.
You could also get away from the town and there would be no invisible walls. There would be only one good ending (where you escape the town safely) and a lot of other bad endings.
For example, if you got away from the town through the woods, there would be a cutscene that would vary 1 would be a hunter shooting you and getting you to his cabin, butchering and roasting you. 2 would be you get devoured alive by wolves/wild dogs. 3 would be devoured alive by a bear.
